Awards based on scholarship, number of courses completed and financial need. A limited number of positions available on a semester by semester basis.

Application for Combined Program
For the BA/MA program

Application to Request Graduation
Completion of form required for graduation. Advisor signature required. Deadlines apply.

Combined Program Registration
Registration form for Combined Program students.  Signatures required.

Confidentiality Agreement
Completion of form required by all students performing an assistantship.  

Independent Study
Completion of form required for requesting an independent study. Signatures required. 

Spouse and Dependent Verification Form

Transcript Request
Completion of form required for requesting a transcript. Fee applies.

Transfer Graduate Course
Application for transfer of credits from another institution. Signatures required. Official transcript and course syllabus required.

Tuition Discount Verification
With verification of employment, the student may request a tuition discount. Deadlines apply.

Course Interruption Overview
Overview of policies & procedures for interrupting/ not completing a course or program.

Change of Program
Application to change a previously indicated academic program of study.

Course Withdrawal
The form is to be utilized after "Drop/Add" period. Grade of "W" appears on the transcript.

Course Withdrawal After Deadline
A student who finds it necessary to withdraw from a course after the deadline for withdrawal has passed. Signatures required. 

Leave of Absence Request
A student who finds it necessary to interrupt his/her course of study for up to three consecutive terms must request a leave of absence.
